You can post documents and forms to the office but they may take longer to process.

Joiner pack
Your new employer should tell you about completing the joiner pack when you join the scheme. They should provide you with a link to it. If you have not already completed it, you can complete it here. It includes 2 online forms:
- Expression of wish for payment of death grant lump sum - let us know your wishes about your nominees.
- Personal Details Declaration - your personal details and, if you want to, express an interest in transferring previous pension rights within 12 months of joining.
Transferring in previous pension rights
Your new employer should tell you about the Personal Details Declaration form to complete when you join the scheme. The form includes an option to express an interest in transferring previous pension rights into the scheme within 12 months of joining. The Personal Details Declaration form is contained in the Joiner pack
Change of home address or email
Notify us of your change of home or email address. Alternatively, you can change your address yourself through MyPension Online. Create an account or log in to MyPension Online
Change of personal details
Notify us if you have changed your title, name or partnership status.
Cohabiting partner
Your cohabiting partner may be entitled to a survivors pension when you die, if they meet certain criteria. You do not need to complete a form to nominate your cohabiting partner. We will obtain the relevant information at the time of your death.
Expression of wish for payment of death grant
As a current member of the LGPS you can nominate any person, company, charity, club or society to receive a payment of 3 times your pay in the event of your death in service.

Divorce or dissolution request form
Request information about your pension for divorce or dissolution of civil partnership proceedings. Find out about divorce or dissolution of a civil partnership
Opt in form (PDF, 161.2 KB)
Join the LGPS, move to the 50/50 section , or move from the 50/50 section back to the main section. Complete a separate form for each employment which you wish to opt in or move sections. Return the form to your employer.
Opt out form (PDF, 607.2 KB)
You cannot complete the Opt out form until you have joined the scheme. Complete a separate form for each employment which you wish to opt out of the scheme and return it to your employer. If you are opting out because you are suffering financial difficulties, you may wish to consider opting into the 50/50 section rather than opting out. Find out about the 50/50 section

To contribute to the Standard Life Additional Voluntary Contribution (AVC) scheme complete this application form detailing your investment choices and contribution amount. Return your completed form to us. We will send you an acknowledgement letter on receipt. Find out about in house AVCs

Retirement Declaration form
You must complete this form when you retire. It is not available on this website as it is provided to you by your employer. Your employer will return it to us along with other information required to calculate your pension. If you have not been given a Retirement Declaration form to complete before your retirement date you should contact your employer directly. Find information about the retirement process. Trace membership you may have in the LGPS in Kent.
Death notification form
Report the death of a member of the LGPS. If you have notified Tell Us Once (TUO) and ticked public sector pension, you do not need to complete our form. Watch a short video about how to use TUO to tell organisations about a death just once
